On this form you will enter your business location information. This information will be used in the event that Illinois workNet needs to contact you. It can optionally be displayed on your Illinois workNet Business Branding page. - Enter your business name.
- Enter your business address.
- Enter your city, state, and zip code.
- Enter your main phone number, and any alternate phone number and/or fax number.
- Enter your TTY number. This is the number deaf and hearing-impaired people can use to call you. If you do not have a TTY number, click the "Use Illinois Relay Center 800 Number" and the Illinois Relay Center's number will be filled in for you. The Illinois Relay Center is a free service for residents and businesses in Illinois to use to communicate with deaf and hearing-impaired people.
- Enter your company email address (example: info@yoursite.com).
- Select a person to be your primary contact.
- Enter your web address (example: http://www.yoursite.com/
 ). Note that http:// is required. - If your company is publicly traded, answer "Yes" to the field "This company is publicly traded." Otherwise answer "No."
- You may upload an optional company logo. You must provide a title so that blind and visually impaired people who use a screen reader can have text read to them in place of being able to see your logo. The logo must be 180 pixels (or less) wide, and 200 pixels (or less) high.
