Persons with Disabilities Employment Experience

This survey is for persons with disabilities. The North West Region Inclusion Team and the Coalition for Citizens with Disabilities in Illinois has developed this survey to better understand the experiences of people with disabilities in work settings. We hope to identify roadblocks, and work to remove barriers to employment or volunteerism for all persons with disabilities.
